The risk may come from another prescription
A patient may think of flibanserin as a sexual-desire medication.
But the body reads it as chemistry.
The drug must be absorbed, processed, and cleared. If another medicine interferes with that process, the effect can change sharply. That is why one of the most important safety stories around flibanserin has little to do with sex itself.
It has to do with CYP3A4.
ADDYI prescribing information states that severe hypotension and syncope can occur when flibanserin is used with moderate or strong CYP3A4 inhibitors, and that use in these settings is contraindicated. The same warning applies to patients with hepatic impairment because flibanserin concentrations can rise. (Access Data)
The fluconazole signal was not subtle
Fluconazole is a familiar antifungal. It is used for infections many patients do not consider medically dramatic.
But in the flibanserin interaction study, fluconazole increased flibanserin exposure 7-fold and peak concentration 2.2-fold. Three of 15 healthy women developed hypotension or syncope, and the study was stopped early.
That is the safety point behind Female Viagra fluconazole hypotension syncope.
The dose of flibanserin did not need to change. The exposure changed because another drug changed the body’s ability to clear it.
“Female Viagra” is the wrong mental model
The nickname makes flibanserin sound like an event-based sexual-performance drug.
That is misleading.
Flibanserin is taken once daily at bedtime. Its label says bedtime dosing is used because taking it during waking hours increases risks such as hypotension, syncope, accidental injury, and central nervous system depression. (Access Data)
That makes the medication list central.
A woman taking flibanserin may not connect it with an antibiotic, antifungal, heart medicine, antiviral, grapefruit juice, or herbal supplement. But the label lists strong CYP3A4 inhibitors such as ketoconazole, clarithromycin, ritonavir, and others; it also lists moderate inhibitors including ciprofloxacin, diltiazem, erythromycin, fluconazole, verapamil, and grapefruit juice. The liver is part of the story
Flibanserin is also contraindicated in hepatic impairment.
That warning is not decorative. The label states that any degree of hepatic impairment can significantly increase flibanserin concentrations, which can lead to hypotension and syncope. (Access Data)
This matters because patients do not always know they have liver disease. Fatty liver disease, alcohol-related liver injury, viral hepatitis, medication-related liver injury, and abnormal liver enzymes can all change a prescribing decision.
A desire drug can become a liver-clearance question.
The practical takeaway
Female Viagra should not be treated as a simple libido product.
For flibanserin, safety depends on timing, liver function, sedating drugs, and especially CYP3A4 interactions. A short course of an antifungal or antibiotic can matter. Grapefruit juice can matter. HIV therapy can matter. Cardiac drugs can matter.
The better question is not only whether low desire fits the diagnosis of HSDD.
The better question is whether the patient’s medication list allows flibanserin to be used safely at all.
